Literal
whole heart, whole mind
Figurative
Heart and soul; doing something wholeheartedly and without reservation.

Where does 全心全意 come from?

It does not come from a specific tale; it is a set phrase built from two parallel pairs (全心 "whole heart" and 全意 "whole mind, whole intent") that reinforce each other to stress total commitment. It is a modern expression that spread during the twentieth century, especially in political and public-service language, where it often appears in the formula 全心全意为人民服务 ("to serve the people heart and soul"). Today it is in fully everyday use.
